How it works

Mechanism of action

Exosomes do not replace cells. They carry signals that change how existing cells behave — reducing inflammation, stimulating repair, and modulating gene expression at the treatment site and beyond.

miRNA Delivery
Exosomes carry microRNA cargo that enters target cells and directly modulates gene expression. A single exosome can carry hundreds of miRNA species, each capable of regulating multiple biological pathways simultaneously — anti-inflammatory, pro-regenerative, anti-fibrotic.

Paracrine Signaling
After fusion with target cell membranes, exosome contents activate downstream signaling cascades. Growth factors (VEGF, TGF-beta, PDGF) stimulate nearby cells to proliferate, migrate, and differentiate into repair-active phenotypes.

Immunomodulation
MSC-derived exosomes shift macrophages from M1 (pro-inflammatory) to M2 (anti-inflammatory, repair-promoting) phenotype. They suppress T-cell proliferation and reduce pro-inflammatory cytokine production — creating a favorable healing microenvironment.

Blood-Brain Barrier Crossing

Exosomes

Exosomes are nano-sized extracellular vesicles (30-150nm) naturally secreted by cells. They function as intercellular messengers — carrying proteins, lipids, and RNA (including microRNA) that modify the behavior of recipient cells. MSC-derived exosomes carry the therapeutic signaling cargo responsible for most of the benefit associated with stem cell therapies, without requiring the live cells themselves.
Exosomes are acellular — they contain no live cells. This eliminates risks associated with cell transplantation such as uncontrolled proliferation, immune rejection of foreign cells, or tumorigenicity. They also show lower immunogenicity than cellular products. However, “safer” depends on the indication — for some applications, the structural and differentiation capacity of live MSCs provides benefits that exosomes alone cannot replicate.
The blood-brain barrier (BBB) is designed to restrict passage of large molecules and cells from the bloodstream into the brain. Exosomes are small enough (30-150nm) and have a lipid membrane composition that allows them to traverse the BBB through transcytosis — the same process the brain uses to transport essential molecules. This makes them uniquely suited for neurological applications where most other biologics cannot reach the target tissue.
Lyophilized (freeze-dried) exosomes are shelf-stable at room temperature for 2+ years. This eliminates the cold chain requirements of frozen products, allows point-of-care storage, and simplifies overnight shipping logistics. Biological activity is fully preserved through the lyophilization process. Upon reconstitution with sterile saline, they are ready for immediate clinical use.
The evidence base is growing rapidly. A 2025 systematic review covering 11 clinical studies found exosome therapy demonstrated improvements in hair density and thickness across androgenetic alopecia and other alopecia types, with no significant adverse reactions reported. ADSC-derived exosomes applied topically showed meaningful results in 39 AGA patients in early clinical work. The current consensus is that exosomes show strong promise but require more large-scale RCTs to establish standardized protocols.
Product selection depends on indication and case complexity. Nano EX (30B) is a starting point for aesthetics and hair. Nano AER (50B) is the standard for most anti-inflammatory and regenerative applications. Nano AER Plus (90B) is for complex or advanced cases. Nano Xsomes (180B/mL) is the highest concentration for neurological or severe inflammatory applications.
